# Configuring User Fields for GoJoin and GoBook Sign-Up Forms

HTML version: [Configuring User Fields for GoJoin and GoBook Sign-Up Forms](https://support.gladstonesoftware.cloud/goconsumer/configuring-user-fields-for-gojoin-and-gobook-sign-up-forms)

## Problem

You need to ask customers extra questions when they join on GoJoin or book on GoBook, such as employer, concession type or emergency contact, and have the answers stored on their record for staff to see.

## Resolution

GoJoin collects name, email, date of birth, phone, address and marketing preference, then payment (and bank details for Direct Debit). Extra questions come from Plus2 **user fields**, added to Operator **form profiles**.

1. **Ensure prerequisites**
   - Confirm the required user field has already been created in Plus2; see [How to Create User Fields in Plus2](../plus2/how-to-create-user-fields-in-plus2.md).
   - Make sure you have administrator access to Operator.

2. **Open Form Profiles in Operator**
   - In Operator, go to `Configuration > Centre Configuration > Form Profiles`, or search "form" in **Configuration**.

3. **Open the relevant profile**
   - Select **Manage** next to the profile you want to configure:

   | Form profile | Used when |
   |---|---|
   | **Sign-up form** (Member sign-up form) | Main profile for GoJoin: existing customers buying another membership from their account |
   | **Guest Sign-up form** | First-time joiners on GoJoin |
   | **My Account Profile form** | Fields shown in the My Account section of Go |
   | **Guest Booking form** | First-time bookers on GoBook |

   - Profiles can be renamed to make them clearer (e.g. "Go Join", "Go Book").
   - The **Guest Booking form** is not needed if everyone must hold a membership to book.

4. **Select form fields**
   - Use the **Select Form Component** dropdown to enable the fields you want to display on the form.

5. **Add user fields**
   - In the **Select Form Component** dropdown, select **User Fields**.
   - From the **Select User Fields** dropdown, choose the field and select **Add**.
   - Fields appear in **reverse order** of adding, so add the question you want last first. Plan the order of the questions on paper beforehand.

6. **Choose which subscriptions show the field**
   - Select **Edit** (the pencil) next to each user field to choose which subscriptions it is visible for. Different subscriptions can display different user fields (for example, student and staff memberships can show different questions).
   - On the right of the screen, enter the subscriptions (Plus2 subscription names or codes) and select **Add**. When all are added, select **Confirm**.

7. **Add eligibility questions to both sign-up forms (if applicable)**
   - Add them to both the **Guest Sign-up form** and the **Sign-up form**, so existing account holders are asked too.

8. **Save the profile**
   - Select **Save** in the top-right corner to apply your changes.

## Notes

- **All fields are mandatory** unless there are multiple telephone number fields. If more than one telephone number field exists, not every telephone field must be completed.
- Only add fields with a clear operational need, such as emergency contact, referral source, photo, membership goal or employer, and add them only on the memberships that need them.
- Answers are stored on the contact record, so staff see them in 360 and can check eligibility at first visit.
- ID numbers collected in a user field aren't copied into the card ID used for barrier access.
- If a user field does not appear in Operator, confirm it was created in Plus2 before configuring it in Operator.
- If the field is not visible on GoJoin or GoBook, check that the correct subscriptions have been added to the field's visibility settings and that the profile has been saved.
